We are looking for a compassionate, detail orientated registered nurse to join our team as a Hospice Registered Nurse (RN). The Hospice Registered Nurse (RN) oversees and coordinates the plan of care for a dedicated case load of patients. This means getting to know your patients, caregivers, and their families while working with a supportive and collaborative interdisciplinary team, focusing on holistic patient-centered care, and quality at end of life.

Responsibilities for Hospice Registered Nurse (RN):
Complete home and facility visits during scheduled hours based upon patient and caregiver needs
Manage comfort measures to ensure quality of life that is based upon the patient’s goals
Coordinate patient care among the hospice team for an assigned case load in a designated territory
Maintain accurate, up to date documentation based on current CMS guidelines
Qualifications for Hospice Registered Nurse (RN):
Active NH RN license
Minimum 1-2 years direct care experience required
End of Life experience preferred
Excellent communication skills necessary
Proof of Flu vaccination
Reliable transportation
Valid driver's license
Automobile insurance
Physical exam within the last year
Two-step TB test
