Advanced RN | Hospice Care SpecialistJoin a dedicated healthcare team providing compassionate hospice care across the Midwest region. As a Registered Nurse specializing in hospice, you will deliver direct patient care using the nursing process, demonstrating clinical expertise and adhering to professional ethical standards.
Essential Duties and Responsibilities- Participate in agency councils, committees, or serve as a clinical coach, supporting continuous professional development.
- Administer medications and perform planned interventions safely and efficiently, following established policies and protocols.
- Communicate patient condition changes effectively to physicians, advocating for patient needs using structured communication methods like SBAR.
- Coordinate patient care collaboratively with healthcare team members and facilities to ensure seamless service delivery.
- Adapt positively to the dynamic healthcare environment and document patient care activities accurately in electronic medical records.
- Evaluate patient progress toward goals and update care plans accordingly, initiating patient-focused plans with family involvement.
- Provide tailored education to patients and families considering their needs, education level, and cultural background.
- Engage in self-reflection to transform experiences into learning opportunities and apply critical thinking in care planning and discharge preparation.
- Maintain up-to-date knowledge and skills through evidence-based practice relevant to hospice nursing.
Graduation from an accredited nursing school is required. A Bachelor of Science in Nursing is preferred.
ExperienceAt least one year of clinical nursing experience is required, with preference given to candidates with home health nursing experience.
Requirements- Current RN license valid in the state or compact licensure required; new graduates must obtain licensure within 90 days.
- American Heart Association Basic Life Support certification required.
- Physical demands include lifting up to 35 lbs, frequent walking, standing, and various movements such as turning, stooping, and reaching.
- Work environment involves exposure to communicable diseases, infectious materials, and varying temperatures.
