Hartford Healthcare seeks a compassionate Hospice Nurse to provide high-quality end-of-life care in home and facility settings. In this field-based role, you will perform comprehensive patient assessments, manage pain and symptoms, administer medications and treatments, and educate and support families during critical moments. You will collaborate closely with an interdisciplinary team to create and update individualized care plans. Provide compassionate hospice nursing care in patient homes and facilities
Perform comprehensive assessments and manage pain and symptoms
Develop, implement, and update individualized care plans
Educate and support patients and families on end-of-life care
Collaborate with an interdisciplinary hospice team for coordinated care
Document care accurately in the electronic medical record
Participate in on-call rotation as needed
