Hartford Healthcare is seeking a compassionate Hospice Nurse to join our Healthcare & Medical Services team. In this field-based role, you will provide expert end-of-life nursing care, pain and symptom management, and emotional support to patients and families in their homes. You’ll collaborate closely with an interdisciplinary team, using cutting-edge tools and evidence-based practices to deliver high-quality, patient-centered care. Provide comprehensive in-home hospice nursing care to adult and geriatric patients.
Perform ongoing assessments, manage pain and complex symptoms, and administer medications.
Develop, implement, and update individualized care plans with the interdisciplinary team.
Educate and support patients and families on disease progression, comfort measures, and end-of-life decisions.
Coordinate services with physicians, social workers, chaplains, and other care team members.
Document all care in the EMR accurately and in compliance with regulatory standards.
Participate in on-call rotations and respond to urgent patient needs.
Advocate for patient comfort, dignity, and goals of care.
Promote a culture of compassion, safety, and continuous improvement.
