Hartford Healthcare seeks a compassionate Hospice Nurse to join our Healthcare & Medical Services team. In this field-based role, you will deliver high-quality end-of-life care, managing complex symptoms, administering medications, and educating patients and families in their homes or facilities. Provide compassionate hospice nursing care in patients’ homes or facilities
Assess, plan, implement, and evaluate individualized care plans
Administer medications and manage complex pain and symptoms
Educate patients and families on disease process and end-of-life care
Document care and assessments accurately in the EHR system
Collaborate with physicians, social workers, chaplains, and aides
Participate in on-call rotation and respond to urgent patient needs
Support families through emotional, cultural, and spiritual needs
Ensure compliance with hospice, safety, and infection control standards
Contribute to quality improvement and interdisciplinary case conferences
