Hartford Healthcare seeks a compassionate Hospice Nurse to deliver expert end-of-life care within our Healthcare & Medical Services department. In this field-based role, you’ll provide in-home nursing care, pain and symptom management, and emotional support to patients and families. You’ll collaborate with an interdisciplinary team, using cutting-edge tools while practicing with autonomy. Provide comprehensive hospice nursing care in patients’ homes and facilities
Conduct holistic assessments and develop individualized care plans
Manage pain and complex symptoms in collaboration with the hospice team
Educate and support patients and families on end-of-life care and options
Coordinate care with physicians, social workers, and spiritual care providers
Document care accurately in the EHR and communicate changes promptly
Participate in on-call rotation and timely response to urgent needs
