Hartford Healthcare seeks a compassionate Hospice Nurse to join our Healthcare & Medical Services team. In this field-based role, you will provide expert end-of-life nursing care, pain and symptom management, and emotional support to patients and families in their homes and facilities. You will complete thorough assessments, develop individualized care plans, and collaborate closely with an interdisciplinary team. Provide compassionate hospice nursing care in patients’ homes and facilities
Perform holistic assessments and manage pain and symptoms at end of life
Develop and update individualized care plans with the interdisciplinary team
Educate and support patients and families on disease process and comfort care
Administer medications and treatments per provider orders and protocols
Document care accurately in the electronic health record
Coordinate services with physicians, social workers, chaplains, and aides
Participate in on-call rotation and timely response to patient needs
Advocate for patient preferences and maintain dignity and comfort
Contribute to quality improvement and best-practice initiatives
