Hartford Healthcare seeks a compassionate Hospice Nurse to provide expert end-of-life care in our Healthcare & Medical Services department. In this role, you will manage complex symptoms, perform skilled nursing visits, and educate and support patients and families in their homes or facilities. You will collaborate closely with physicians, social workers, and the interdisciplinary team to develop personalized care plans. Provide comprehensive hospice nursing care in patients’ homes or facilities
Perform ongoing assessment, pain and symptom management, and medication administration
Develop and update individualized care plans with the interdisciplinary team
Educate and support patients and families on disease process and end-of-life care
Coordinate services with physicians, social workers, and spiritual care providers
Document care accurately in EMR and adhere to regulatory and safety standards
