Description
Hebrew SeniorLife, a Boston-based nonprofit senior care organization affiliated with Harvard Medical School, is seeking a Hospice RN to provide compassionate end-of-life care to older adults in home and facility settings. In this role, you will deliver skilled nursing care, manage pain and symptoms, and support patients and families through education, emotional support, and advocacy. You will collaborate closely with an interdisciplinary team of physicians, social workers, chaplains, therapists, and caregivers to develop and implement individualized care plans that honor each patient’s goals, dignity, and cultural preferences. Responsibilities include comprehensive assessments, medication administration, accurate EMR documentation, and participation in care conferences and quality initiatives. Conduct comprehensive assessments, monitor symptoms, and implement individualized care plans
Administer medications and treatments, including pain and symptom management per provider orders
Educate patients and families on disease progression, comfort measures, and available support services
Collaborate with an interdisciplinary team including physicians, social workers, chaplains, and therapists
Document assessments, interventions, and patient response in compliance with regulatory and agency standards
Participate in care conferences, quality initiatives, and performance improvement efforts
Support patients and families emotionally and culturally, honoring preferences and goals of care
Coordinate transitions of care and communicate updates with caregivers and providers
Precept and mentor new staff, contributing to a supportive, learning-focused culture
Hospice and palliative nursing care

IV and medication administration

Electronic medical record (EMR) documentation
Care planning and coordination
Patient and family education
Interdisciplinary team collaboration
End-of-life communication skills
Regulatory and hospice compliance knowledge

Active Registered Nurse (RN) license in Massachusetts
Hospice and Palliative Nurses (CHPN) certification preferred
