Hospice Case Manager / Admissions Nurse
We are seeking an experienced Hospice Case Manager / Admissions Nurse to provide compassionate, patient-centered care, manage hospice cases, and complete patient admissions. The ideal candidate will have strong hospice experience, excellent clinical judgment, and the ability to coordinate care with patients, families, and interdisciplinary teams.
Schedule:
- Full-time: 40 hours/week
- Monday–Friday, 8:00 AM–4:30 PM
- Rotating on-call coverage: Weekdays 4:30 PM–8:00 AM and weekends 8:00 AM–8:00 AM
- Coverage area: 50-mile radius from office
- Mileage reimbursement provided between patient visits
Responsibilities:
- Manage hospice patient caseloads and complete admissions
- Perform patient assessments and develop care plans
- Provide skilled nursing care and symptom management
- Coordinate with physicians and hospice care teams
- Maintain accurate documentation in PointCare
Requirements:
- Hospice nursing experience required
- Experience with hospice admissions and case management
- PointCare experience preferred
- Current CPR certification
- Strong communication, organization, and clinical skills
