RN – Home Health & Hospice | Roles & Responsibilities
Provide skilled nursing care to patients in their homes for both Home Health and Hospice services.
Perform comprehensive patient assessments and develop individualized plans of care.
Complete OASIS assessments, admissions, routine visits, and hospice evaluations.
Provide wound care, including Wound VAC (NPWT) management and dressing changes.
Perform PICC line dressing changes, port access, IV infusions, catheter insertions, drain management, and other skilled nursing procedures.
Educate patients and families on disease management, hospice philosophy, medications, and infection prevention.
Coordinate care with physicians, LPNs, case managers, and the interdisciplinary care team.
Draw laboratory specimens, maintain accurate Epic Remote documentation, and monitor patient progress.
Travel between patient homes, participate in on-call rotations, and provide weekend/holiday coverage as scheduled.
Required Documents / Certifications
- Active South Dakota RN License
- BLS
- Valid Driver's License
- 1.5+ years of Home Health and Hospice RN experience
- OASIS experience
- Experience with wound care, Wound VAC, PICC lines, port access, IV infusions, catheter insertion, and drain management
- Ability to drive for field visits, including at night
