RN – Home Health Hospice (Martinez, CA) / Active CA RN License + BLS required / 1+ year of recent Home Health or Hospice experience (within the last 3 years) / 13-week contract / Day shift (8:30 AM–5:00 PM), 5x8s, 40 hrs/week / Every other weekend required / In-office role processing Home Health and Hospice referrals for Northern California / Health Connect (Epic) charting required / Minimal travel (~10%) / Experience with referral coordination, case review, and Home Health/Hospice workflows preferred.
